If neither of the parents is eligible for the Taiwan Household Registration Transcript (HRT), please submit your child’s original hospital birth certificate.
The original hospital birth certificate must meet the following requirements:
- It bears the official seal of the hospital/clinic and the date of issuance;
- It bears the signature of the attending physician. If your child’s birth certificate is not signed by the attending physician, the job title of the authorized signer must be listed; and
- It is written in the English language.
- Please bring one original and one photocopy.
Please note: (Names to be used)
- The names of the child and the parent(s) in the hospital/clinic birth certificate must be the same as the names written in the Form DS-2029 and on the valid photo IDs presented.
- All the names listed on the hospital/clinic birth certificate should appear as follow:
- Given name followed by middle name and last name
(for example: Jane Mei-Yun DOE); OR - Last name, given name followed by middle name
(for example: DOE, Jane Mei-Yun) (Note the comma after the last name).
- Given name followed by middle name and last name
- The child’s name in English listed in hospital/clinic birth certificate is the name to be used in your child’s Consular Report of Birth Abroad and U.S. passport.
- Non-U.S. citizen parent: the English name listed in your child’s hospital/clinic birth certificate must be your full name in your passport.
- U.S. citizen parent(s): the English name listed in your child’s hospital/clinic birth certificate must be your full name in your U.S. passport.
