You must appear in person to report a lost or stolen passport and apply for a replacement.

 

Important Notice:

A full-validity passport can only be issued when you have submitted sufficient evidence of citizenship AND identity.  If you cannot submit evidence of citizenship AND identity, you will be issued a limited-validity passport.

Acceptable evidence of citizenship:  Certificate of Naturalization/Citizenship, previous expired U.S. passport, U.S. issued birth certificate, and Consular Report of Birth Abroad.

Acceptable of evidence of identity:  U.S. driver license, non-driver state issued ID, previous expired U.S. passport, and valid foreign passport.

You are required to submit the following:

1. Form DS-11(Online Form Filler): Please bring this completed, unsigned form to your appointment. The Consular Officer will ask you to sign this form during your appointment.
2. Form DS-64 (PDF 51.03KB):  Statement Regarding Lost or Stolen Passport.  Complete and print the form.
3. Evidence of U.S. Citizenship:  Such evidence may include but is not limited to:  U.S. birth or naturalization certificate, Consular Report of Birth Abroad or an expired U.S. passport.
4. Valid government issued photo ID such as a U.S. or foreign passport, driver’s license, or military ID.
5. A Social Security Number (SSN) is required for all passport applications.  You must have your SSN ready when completing the form.
   Please Note:
   If you do not have your SSN on hand or do not recall the number, click here to learn more about obtaining your SSN. Please do not schedule an appointment for passport application until you have contacted the Social Security Administration in Manila to confirm whether or not you have an SSN.
6. One photograph (2 x 2 inch):  Click here for more information on passport photo requirements.  Photos which do not meet the requirements will be rejected.
7. Applicants under 16:  In addition to Form DS-64, please see instructions on Minor Passport.
8. NIA Report: Written report of lost or stolen passport from the nearest National Immigration Agency (Required to depart Taiwan).

10. The processing time is 3 – 4 weeks. Your new and previous passport will be returned to you via courier for a fee of NT$190, pay on delivery.
11. Emergency Travel: If you have an emergency travel need and can provide proof of a paid itinerary for travel in less than one week, you may request for a limited validity emergency passport, the Consular officer will determine if you qualify for an emergency passport on a case by case basis.
12. Taiwan Exit Permit: Since you have been issued a new passport which has a different passport number than the one you used to enter Taiwan.  When exiting Taiwan at the airport, the National Immigration Agency of Taiwan will ask you to complete forms to issue an Exit Permit. An Exit Permit is required in order to leave Taiwan using this new passport.
