You are eligible for a no fee “re-application if you meet the requirements to use form DS-5504.  If you are using form DS-5504, then you can apply by mail. If you are using applications forms other than DS-5504, you are required to submit your application in person through a pre-arranged appointment.

Apply by Mail:

The following items are required:

1. Form DS-5504 (PDF 89.33KB): Please complete and sign on form including Taiwan contact telephone and mailing address.
2. Your most recent U.S. passport: Original and 1 photocopy of the photo and signature page.
3. One photograph (2 x 2 inch): Click here for photo requirements. Photos which do not meet the requirements will be rejected.
4. A Social Security Number (SSN) is required for all passport applications.  You must have your SSN ready when completing the form.
   If you do not have your SSN on hand or do not recall the number, click here to learn more about obtaining your SSN.If you have never applied for an SSN, you still need to contact the Social Security Administration Office in Manila to obtain a confirmation email that an SSN was never assigned to you.Please do not schedule an appointment for passport application until you have contacted the Social Security Administration in Manila to confirm whether or not you have an SSN.

6. There is no fee.
7. U.S. Passport Delivery Form. Please complete and print the form.  Your new passport and previous passport will be delivered to you by courier for a fee of NT$190, pay on delivery.
8. The processing time is 3 – 4 weeks. Your new and previous passport will be returned to you via courier for a fee of NT$190, pay on delivery.
