Your application package should contain the following documents:

 

  1. Loss of Nationality Questionnaire (PDF 664KB)
  2. DS-4080 (PDF 28KB)
  3. DS-4081 (PDF 14KB)
  4. Last issued U.S. passport
  5. Foreign valid passport(s)
  6. Original Certificate of Naturalization / Certificate of Citizenship
  7. Original Consular Report of Birth Abroad / U.S. issued birth certificate
  8. Written explanation of your reasons for renouncing (if applicable)
  9. Evidence of name change (if applicable)
  10. Email your complete application package to TaipeiCLN@state.gov.  Please allow 7-10 business days for us to review your documents.  We will contact you via email.
